Our Vision
We aspire to enable everyone who lives on Portland to realise their potential to live healthier, happier and more fulfilling lives, so that they can start well, live well and age well.
As a team, we are invested in increasing quality and satisfaction with healthcare on the island. We recognise the power of community passion and want to harness this, working alongside the community and our colleagues within the Primary Care Network to make our vision reality.
Our mission sets out how we intend to achieve this vision.
In order to provide a high quality primary care service we will:
- Provide patients with chronic diseases with excellent evidence-based care
- Treat all patients, carers and staff with dignity, respect and honesty.
- Understand and be committed to our patients’ needs
- Be courteous, approachable, friendly and accommodating
- Be a patient centred service through shared decision making and good communication
- Safeguard children and adults by adopting best practice and local guidelines and pathways
- Encourage patients to maintain a healthy lifestyle and access screening services
- Act with integrity and complete confidentiality
- Ensure safe and effective services
- Promote continuity of care so far as is possible
- Maintain the motivation skills and spirit of our Practice team
- Monitor and auditing our practice and patient feedback to improve our healthcare services
- Maintain high quality of care through continuous learning and training
- Operate in the spirit of and in accordance with the legislation of diversity and equality
- Improve access for local people to the resources they need to lead healthier lives through a strengthened and well-connected network.
- Secure as many services as possible to provide care on the island or nearby to ensure we are removing barriers to stop our community being disadvantaged
- Ensure effective and robust information governance systems
- Build an organisation that is collaborative, effective, responsive, safe, efficient and well led, with a sense of partnership with our local community, clinical and social care partners.
